What is SLT?

Selective Laser Trabeculoplasty (SLT) is a minimally invasive laser procedure used to lower intraocular pressure (IOP) in patients with open-angle glaucoma. The laser targets pigmented cells in the trabecular meshwork, the eye’s natural drainage system, to increase fluid outflow and decrease eye pressure. SLT’s “selectivity” means it precisely treats targeted tissue without damaging surrounding structures, making it repeatable if necessary. It’s an effective alternative or addition to glaucoma medications.

SLT is designed to improve the outflow of the fluid inside your eye (aqueous humor), lowering the buildup of pressure that can damage the optic nerve in glaucoma.

SLT only impacts pigmented cells and causes no permanent injury to the trabecular meshwork, unlike older laser procedures.

How Does SLT Work?

SLT enhances the eye’s natural drainage system using targeted laser energy, prompting a healing response.

The laser selectively stimulates specific cells. The body’s immune response then improves drainage function over time.

Because SLT does not burn or scar tissue, it preserves the option for future treatments.

Who Can Benefit From SLT?

SLT is appropriate for many glaucoma patients wishing to improve eye pressure control.

SLT is often recommended early for open-angle glaucoma to limit the risk of progressive damage.

Those who have trouble using or tolerating eye drops may find SLT a welcome option.

Patients preferring to minimize their reliance on daily eye drops may benefit from SLT.

  • Primary open-angle glaucoma
  • Pigmentary glaucoma
  • Pseudoexfoliation glaucoma
  • Juvenile open-angle glaucoma

Benefits of SLT

SLT provides many advantages in glaucoma management.

The SLT procedure is performed in the office and typically causes little or no discomfort.

Most patients experience a significant drop in intraocular pressure.

Most people can return to activities by the next day.

Because the procedure does not scar tissue, it may be repeated if pressure rises again.

Some patients can use fewer or no glaucoma drops following successful SLT.

The SLT Procedure: What to Expect

SLT is a quick office procedure with minimal discomfort.

The treatment is done in our clinic, hospitalization is not needed.

There are no injections; numbing drops are used for comfort.

A lens with lubricating gel is placed on your eye to direct the laser.

Bright flashes and clicking sounds are common sensations during treatment.

The SLT treatment takes about 5–10 minutes per eye.

Slight blurring from the gel is expected but resolves quickly.

Most patients resume daily routines the next day.

Risks and Side Effects

SLT is very safe. As with any procedure, some side effects can occur.

  • Mild discomfort and redness
  • Temporary blurred vision

Occasionally, eye pressure may spike for hours to days, usually monitored and easily treated.

  • Ocular inflammation (iritis)
  • Very rarely: bleeding (hyphema) or corneal changes

SLT does not restore vision already lost to glaucoma; it helps prevent further progression.

Results and Long-term Outlook

SLT can successfully control intraocular pressure for many patients.

Approximately 70–85% of patients benefit from a significant drop in eye pressure after SLT.

The effects generally last 2–3 years, and treatment can be safely repeated if needed.

Some patients may still need eye drops or other interventions for optimum control.

Is SLT Right For You?

SLT is a safe, effective glaucoma treatment, often considered as an initial therapy or when eye drops aren’t tolerated. Speak with our doctors to explore your individualized options at Alden Leifer, MD and Associates. We’ll help you decide if SLT is suitable for your situation and needs.

Frequently Asked Questions

Get answers to common SLT questions below.

The full effect of SLT treatment may take 4 to 6 weeks to develop. Your eye pressure will be monitored closely during this period.

SLT is typically performed on one eye at a time so your doctor can monitor response and reduce the risk of complications. If both eyes need treatment, the procedures are usually scheduled a few weeks apart.

SLT can often reduce or occasionally eliminate the need for glaucoma drops, but not always permanently. Some people will still need medications to maintain their target pressure.

SLT is generally painless. Numbing drops are used, and most patients experience only slight pressure or mild irritation, with significant pain being very uncommon.

Patients should avoid rubbing their eyes and should use prescribed drops as directed. Most can return to normal activities immediately, but follow-up visits are important for pressure monitoring.

Yes. Because SLT does not cause tissue scarring, it can be safely repeated if eye pressure rises in the future.

SLT is most effective for open-angle glaucoma types, including primary open-angle, pigmentary, and pseudoexfoliation glaucoma. It is not used for angle-closure glaucoma.

If SLT alone is insufficient, additional therapy such as medications, other laser procedures, or surgery may be recommended.

SLT is considered very safe, with no permanent tissue damage expected. Temporary mild side effects may occur, but serious complications are rare when managed by experienced professionals.

Frequent follow-up appointments are important: typically a few days after the procedure, then at one month, and then every 3–6 months, or as your doctor recommends.
